“ATTENTION TEAMS WANTING SPONSORSHIP”

If you are a team Captain and are looking for a sponsorship please fill out the Team Application and Team Roster (in full).

Once this is done Fax it back or email it back to us and we will look it over and contact you for eligibility. You do not have to fill out the Credit Card part until you order, but we have to have that part filled out in full before we send you the product.

Please include a history of your team including your past games played and the games you plan on attending in 2007.ect...
